This is common, and there are various reasons why. The outer casing usually dissolves after the stomach, especially if taken with a meal. This might be necessary as the contents may be more/most effective if they are absorbed in the small intestine, rather than the stomach, especially if the medication is negatively impacted by stomach acid. It can also just act as a preventative for making you nauseous. The two tablets are likely the same base medication with different release times. For example, the small tablet might be fast acting, and give your body a big spike in medication so you feel better/start doing its job in overdrive. A problem with a fast acting drug is that it’s also fast to dissipate, so a secondary tablet is added that dissolves slowly and gives your body a shallower release curve. When you overlap/combine the two curves on a graph, you’ll see a big spike up front, the spike start to fall off, then rise again slowly, never reaching as high as the spike, but really drawn out like a speed bump, that way the medication lasts for several hours.

It is Expro D 30/40 SR which is a combination of two different drugs, domperidone (30mg) + esomeprazole (40mg), combined in one capsule. The capsule is just there to hold a tablet of each drug. Since each drug was probably approved on it’s own in the tablet form before the combination was approved this is a simple and cheap way to make it easy to take. Since it is labeled SR this suggests at least one tablet is sustained release which would generally mean that they are not chemically compatible to be mixed together. Very possible both drugs were developed by the same company and were often used together. Getting them approved as a single dose unit will allow them to extend the patent. One generic domperidone (30mg) tablet and one generic esomeprazole (40mg) might be cheaper than one Expro D but you pay for “convenience” of only having to take one capsule and you don’t have to worry about forgetting to take one or the other.
